A trip to the Cayman Islands should be all about relaxation and adventure, not worrying about mosquito-borne illnesses. However, as dengue fever remains a concern in many tropical regions, travelers are asking: how significant is the risk? According to the Centers for Disease Control and Prevention, as of April 1, 2026, the Cayman Islands meets the risk classification criteria for frequent or continuous risk of dengue fever (defined as more than 10 locally acquired cases in at least three of the previous ten years).1 One of the Cayman Islands’ closest neighbors, Jamaica, has reported substantially higher case numbers of dengue. According to the Jamaican Ministry of Health and Wellness, in 2024 there were 1797 suspected or confirmed cases of dengue with 2 deaths, 491 cases with 0 deaths in 2025, and 62 cases with 0 deaths as of March 12, 2026.3,4,5
Dengue is a viral infection spread by mosquitoes, and is especially prevalent in the Caribbean region. Mosquitoes become infected after biting an individual in the first week of infection, and luckily the virus cannot be spread directly from person-to-person.2 While the risk of dengue fever is present in the Cayman Islands, it can be effectively managed with the appropriate preventative measures; protection begins with minimizing exposure to mosquito bites.
How to Protect Yourself
The Cayman Resident health advisories explains, “dengue is not currently categorized as endemic to the Cayman Islands, however, it is still wise to avoid being bitten by a mosquito and to familiarize yourself with the symptoms should you become sick.” Common symptoms of dengue fever include:
- High Temperature, or feeling hot/shivery
- Severe headache
- Pain behind the eyes
- Muscle and joint pain
- Feeling or being sick
- Widespread red rash (may not be visible on darker skin tones)
- Stomach pain and loss of appetite
Those most at risk for developing severe dengue are those who have had the infection before, pregnant women, and infants. Signs of severe dengue include:
- Severe stomach pain
- Swollen stomach
- Persistent vomiting, and vomiting blood
- Bleeding gums or bleeding under the skin
- Breathing difficulties or fast breathing
- Cold, clammy skin
- A weak but fast pulse
- Drowsiness or loss of consciousness
About 1 in 4 people infected with dengue will develop symptoms; however, asymptomatic individuals may still transmit the virus if bitten. Preventative strategies are therefore essential. Recommended measures include the use of effective insect repellents, wearing loose-fitting protective clothing (as mosquitoes can bite through tight fabrics), limiting exposure during peak mosquito activity (typically late afternoon and evening), staying in air-conditioned or screened-off environments, and avoiding areas with standing water where mosquitoes breed. Consistency in these protective behaviors, not just occasional use, is critical to avoiding mosquito bites. Simple steps make a big difference, and long-lasting protection is critical in the Cayman Islands.2
